You may update your beneficiaries by submitting a hard copy of your new SLOCPT Beneficiary Designation form with your wet signature. The following restrictions apply:
If you are married or in a Registered Domestic Partnership (RDP): Your spouse/RDP must be named your sole primary beneficiary. You may name someone else provided your spouse’s/RDP’s signature is notarized.
If you are now divorced from your current designated beneficiary on file: You must first submit copies of your filed divorce documentation for review. Upon determination by SLOCPT that all necessary divorce documentation has been received, a new Beneficiary Designation form will be issued to you for submission.
